New research suggests total abstention and excessive drinking are both risks

People who abstain from alcohol or consume more than 14 units a week during middle age (midlife) are at increased risk of developing dementia, finds a study in The BMJ today.
Admissions for alcohol related chronic diseases and cases of dementia from 1991, and the role of cardiometabolic disease were then identified from hospital records.
"People who completely abstain from alcohol may have a history of heavy drinking and this can make it hard to interpret the links between drinking and health".
They noted that with every increase of seven units per week there was a significant 17 per cent increase in dementia risk.
This appears to confirm the findings of a variety of reports over time on alcohol consumption, health and life expectancy, which have suggested the evidence is weighted towards what is described as a J model, whereby no alcohol consumption is less beneficial than moderate consumption, but risk rises again with increasingly heavy consumption.
The study affirmed the earlier finding that high alcohol consumption (drinking more than the recommended 14 units of beer, equivalent to six pints of average-strength beer per week) is linked to an increased risk for the illness.
The research, led by Severine Sabia at the French National Institute of Health and Medical Research, also found that - among moderate drinkers - wine consumption correlated with a lower risk of dementia than beer or spirits, such as whiskey, gin or vodka.
The latest follow-up found 397 recorded cases of dementia.
Perhaps the most important contribution of the recently published research, however, was the discovery that doing the exact opposite of excessive drinking - abstinence from alcohol consumption in middle-age - is a factor for developing the illness as well.
"Future research will need to examine drinking habits across a whole lifetime, and this will help to shed more light on the relationship between alcohol and dementia", Sara Imarisio, head of research at Alzheimer's Research UK, told the Science Media Centre.
In the United Kingdom, 14 units of alcohol a week is now the recommended maximum limit for both men and women, but many countries still use a much higher threshold to define harmful drinking.
She added: 'Current alcohol guidelines recommend not regularly drinking more than 14 units a week for both men and women, and drinking over this amount impacts on a number of health conditions, not only dementia.
"A good motto tends to be, what is good for your heart is good for your brain".
But the reasons why abstinence and excessive drinking are harmful are thought to be different.
